Please Note: Please be punctual for this exam as it may last 1 to 2 hours. You will be seen by Dr. Ernesto Villafuerte who is using the office called Orthopedic Evaluations. Parking is complimentary in the Winchester Plaza Parking lot. Be sure to park in the appropriate spaces. Winchester Plaza has two rows of offices, separated by a parking lot and Doctor’s office is located at the back row, right hand corner of Winchester Plaza. Winchester Plaza is located on Winchester Boulevard, between Williams and Payne Street.

From Salinas (approx. 65 miles)
Take CA-85 North Mountain View
Travel about 15 miles on CA-85 and then take CA-17 North to San Jose
Exit at Hamilton Avenue and turn left at the stoplight onto Creekside Way
Turn left onto Hamilton Avenue and continue for approx. 3/4 miles till you will arrive at Winchester Boulevard
Turn right onto Winchester Boulevard and continue for approx. 1/2 mile
Drive pass the Bethel church
Continue to Williams Street and make a U-turn on Winchester Boulevard & Williams Street Turn right into the driveway of Winchester Plaza

From San Francisco (approx. 50 miles)
Take US-101 South to CA-85 South
From CA-85 South, take I-280 San Jose for approximately 5 miles
Exit at Winchester Boulevard
Turn left at the stoplight onto Moorpark Ave and get immediately to the far right lane
Continue for approx. 50 feet and turn right onto Winchester Blvd
Continue for approx. 1/2 mile pass Williams Street. Then, continue for another half block on Winchester Blvd
Turn into second driveway of Winchester Plaza

From Oakland (approx. 45 miles)
Take I-880 South towards San Jose
Exit at Stevens Creek Boulevard
Turn right on Stevens Creek
Pass Valley Fair Shopping Center (on your right) to Winchester Boulevard
Turn left onto Winchester Boulevard. Continue for approx. 1 1/2 miles or about 6 stop lights.
Pass the last stop light at Williams Street and continue another half block on Winchester Blvd
Turn into the second driveway of Winchester plaza (on your left) just past the Guarantee bank

From Santa Cruz (approx. 30 miles)
Take CA-17 North toward San Jose/Oakland
Exit at Hamilton Avenue and turn left at the stoplight onto Creekside Way
Turn left onto Hamilton Avenue and continue for approx. 3/4 miles till you will arrive at Winchester Boulevard
Turn right onto Winchester Boulevard and continue for approx. 1/2 mile
Drive pass the Bethel church
Continue to Williams Street and make a U-turn on Winchester Boulevard & Williams Street Turn right into the driveway of Winchester Plaza

From Castroville / Monterey (approx. 55 miles from Castroville or 70 miles from Monterey)
Take CA-156 to US-101
Take US-101 to CA-85 North Mountain View to CA-17 North to San Jose
Exit at Hamilton Avenue and turn left at the stoplight onto Creekside Way
Turn left onto Hamilton Avenue and continue for approx. 3/4 miles till you will arrive at Winchester Boulevard
Turn right onto Winchester Boulevard and continue for approx. 1/2 mile
Drive pass the Bethel church
Continue to Williams Street and make a U-turn on Winchester Boulevard & Williams Street Turn right into the driveway of Winchester Plaza

From San Jose International Airport (approx 6 miles)
Proceed South on Airport Blvd towards Coleman Ave
Turn Left on Coleman Ave
Merge onto I-880 South towards I-280/Santa Cruz
Exit at Stevens Creek Boulevard
Turn right on Stevens Creek
Pass Valley Fair Shopping Center (on your right) to Winchester Boulevard
Turn left onto Winchester Boulevard. Continue for approx. 1 1/2 miles or about 6 stop lights.
Pass the last stop light at Williams Street and continue another half block on Winchester Blvd
Turn into the second driveway of Winchester plaza (on your left)
